Ambulatory Care Service (DACS) Unit, 42 S |
DIABETES AMBULATORY CARE
SERVICE (DACS):
- Ambulatory (if possible) stabilisation and education of
newly diagnosed infants, children and adolescents aged <19 years
with type 1 and type 2 diabetes
- Inpatient diabetes care for 0-19 year olds
- Continuing ambulatory diabetes care for individuals aged 0 –
25 years
All diabetes care provided is age-appropriate evidence based best
practice |
ENDOCRINOLOGY:
Diagnosis and management of:
- Short stature
- Disorders of puberty (precocious/delayed )
- Thyroid/Adrenal/Pituitary Disorders (including congenital
hypothyroidism identified on Newborn Screening)
- Obesity
- Accelerated growth/Tall stature
- Disorders of Calcium Metabolism and Metabolic Bone Disease

MODELS OF CARE:
1] AMBULATORY CARE (42S):
Predominantly for diabetes ambulatory care (see above) but also some endocrine
ambulatory care e.g. initial management of neonatal hypothyroidism, other brief
semi-urgent endocrine consultations. DACS is geographically located in 42 S and
provides telephone and direct access to a paediatric endocrine advanced training
registrar, an SRMO, and diabetes nurse specialists between 0800 and 1700hrs on
week days (public holidays excluded), and paediatric endocrine consultants as
required and as available. Paediatric Endocrine Consultants are rostered to
provide 24 hour On Call consultant support.
2] INPATIENT:
- Emergency inpatient care (including inpatient consultations
for other Units) for endocrinology and diabetes patients aged <
19 years
- Neonatal Endocrine NICU ward consultations
- Endocrine testing e.g. Growth hormone arginine/glucagon testing, water
deprivation tests
- Bisphosphonate infusions

OUTPATIENT:
i] Paediatric Diabetes Multidisciplinary Outpatient Clinics are held in Clinic A
OPD weekly on Friday mornings, except on the 1st Friday (when paediatric
endocrine clinics are held). 30 – 40 children under 15 years of age are seen per
clinic, and are attended by all DACS clinical staff, together with a biochemist
to perform the 3 monthly HbA1C’s
ii] Young Adult Diabetes (YADS) Multidisciplinary Outpatient Clinics (CLINIC
BOOKINGS THROUGH DACS RECEPTION) are held in Clinic A OPD twice monthly on 1st
and 3rd Monday evenings. This is a transition clinic. 40 – 45 young people aged
15 – 25 years are seen per clinic, and are attended by 2 paediatric
endocrinologists and 3 adult endocrinologists, paediatric or adult diabetes
registrar, paediatric and adult nurse educators, dietician, paediatric (DACS)
social worker and a biochemist to perform the 3 monthly HbA1C’s
iii] Paediatric Endocrinology Outpatients run once to twice monthly Monash
Medical Centre is one of the two paediatric “Growth Centres” in Melbourne (the
other being RCH). We co-ordinate Federal Government regulated growth hormone
treatment and contribute to the National Growth Hormone Database “OZGROW”.
Generally the more complex paediatric endocrine conditions are seen in this
clinic.
iii] “Fee for Service” Paediatric Endocrine Clinic has been established to
manage increasing numbers of paediatric endocrine patients. Patients requiring
minimal allied health support are preferentially seen in this FFS clinic, which
are held weekly on Monday afternoons in Special Medicine at MMC.
iv] Metabolic Bone Disease Clinic - This clinic runs on the 4th Thursday morning
of each month in Clinic A outpatients, adjacent to the Paediatric Orthopaedic
Clinic which runs in Clinic B OPD, and children and adolescents with complex
metabolic bone disease or disorders of calcium metabolism are managed within
this clinic. Subspecialists attending this clinic include paediatric orthopaedic
surgeons, a paediatric nephrologist, paediatric rheumatologist and paediatric
endocrinologists. |

OUTREACH CLINICS:
MMC Paediatric Endocrinologists provide privately funded
outreach services to the Gippsland Corridor and Peninsula Hospital. |
